As recently reported in the Leicester Mercury, Leicester Hospitals have experienced eight ‘never events’ over the past 15 months, all of which have occurred at University Hospitals of Leicester NHS Trust.
These failings were revealed after the Leicester City Clinical Commissioning Group (CCG) published details of never events across Leicester Hospitals last year.
‘Never events’ are preventable safety incidents that should not occur if the available preventative measures are implemented.
The CCG uncovered never events ranging from a man being mistakenly circumcised, instead of having a cystoscopy, the wrong patient being operated on and a swab being left in a child after a surgical procedure.
Moira Durbridge, Director of Safety and Risk at Leicester’s Hospitals said: “We remain deeply and genuinely sorry to those patients involved, and are committed to learning and improving and have enshrined this work into our clinical priorities within our Quality Strategy for 2019/20.”
The CCG noted an increase in never events in Leicester hospitals and expressed concern that there was a “failure to demonstrate learning from never events”
